Report on the draft Health Service Safety Investigations Bill is published

The House of Lords and House of Commons created a Joint Committee to review the draft Health Service Safety Investigations Bill.  This Committee was appointed to gather evidence and give feedback on the draft Bill.  CHFG consulted with experts and collated constructive feedback during this process.  We are pleased that the Joint Committee took an evidenced-based approach and listened to a range of experts including CHFG supporters.
The priority of HSSIB investigations will not seek to apportion blame, but to set up a 'safe space'.  This will prioritise the consideration of ‘human factors’ as causes of the failures it identifies in the provision of healthcare. We hope to see this expanded to cover Wales, Scotland and Northern Ireland.

Health Service Investigation Branch publishes first two reports

The first relating to a surgical never event of wrong prosthesis where incompatible components were used. The second investigation reviewed the transition of care, from child and adolescent, to adult mental health services.  The investigation was designed to understand how variations in the transition, impacts the safe and effective care of young people. 
As a result of these investigations the Healthcare Safety Investigation Branch identified key findings and recommendations.

We're  bringing together designers, clinicians & procurement

The CHFG will be running a number of events starting in September working with the Royal College of Art, Helen Hamlyn Centre for Design.  We want to understand – how can we encourage healthcare to procure design that improves safety?  More details to come.  We need to raise sponsorship to support this event.  Please contact for more information.
