Florida Center for Inclusive Communities (FCIC) Needs Assessment Survey

Greetings,
The Florida Center for Inclusive Communities is conducting an important needs assessment about services and supports for Floridians with developmental disabilities (DD) throughout Florida. We invite responses from people with DD, family members and caregivers of people with DD, and providers for people with DD. The survey will take about 10 minutes, is available in both English and Spanish, and can be completed on a computer, smartphone, or tablet. Your participation is voluntary and confidential.

Background:

The Florida Center for Inclusive Communities (FCIC) is a University Center for Excellence in Developmental Disabilities (UCEDD) for Education, Research, and Service. We are one of 68 UCEDDs throughout the USA, funded by the Administration on Intellectual and Developmental Disabilities. FCIC is based at the University of South Florida in Tampa. We undertake research, evaluation, education, training and technical assistance activities in areas including Early Childhood, Education, Employment, Community Supports, Health, and Interdisciplinary Training.
