South Georgian Bay Ontario Health Team partners continue to work together to achieve our goals and priorities. In this edition of our e-newsletter, we are thrilled to share our Annual Plan with you that showcases where we are heading and our current projects, including how we are redesigning access to services so everyone in South Georgian Bay can get the help they need when and where they need it.

Integrating Care in South Georgian Bay 

Using Digital Technology to Improve Access to Services

In South Georgian Bay, we look at the health and social system as one interconnected system. Our areas of focus were developed together with our community and are based on what we heard was most important for patients and families in our region.

We're working together with our community to provide you with the following tools to address local health needs in South Georgian Bay to improve health outcomes. This means looking at all the factors that contribute to a healthy community, including the Social Determinants of Health. 

Social Determinants of Health can significantly impact health outcomes, especially for vulnerable populations. For example, some community members may be so focused on securing basic needs, like housing and food, that their ability to focus on healthy eating or healthy behaviours can be impacted. 

That's why we've been busy developing resources for you to ensure you know where to get the support you need for your health, mental health and everything else in between.

24/7 Access to Services

Information about Local Health and Social Services Anytime You Need It

It can sometimes be difficult to know where to go for support for health care, social services (including mental health and addictions services) and other resources available in our South Georgian Bay Community.

That’s why we’ve developed an online resource to help connect you to the services you need. And if you have any questions, just reach out. We are here for you.

Community Information Exchange (CIE) 

Connecting Your Care Teams to Improve Your Health

The South Georgian Bay Community Information Exchange (CIE) in collaboration with Community Connection 211 is the first of its kind in Canada. It's a very exciting step forward in how information is combined to make accessing health and social care in South Georgian Bay easier for you. 

The CIE will create a digital record focused on the patient and shared with both 211 and the patient's circle of care, so everyone involved in the patient’s care will understand all the things that are impacting the health and social care needs of the patient. The patient will not have to re-tell their story and will receive all necessary support to stay healthy and connected in their community. 211 is a free 24/7 helpline answered by Community Navigators, who help callers connect to essential resources like housing and income support. 

How Do I Access the CIE?

If you are accessing health and social care in South Georgian Bay, you will automatically have the option of having your information become available to your family doctor. 

If you do not have a family doctor, you can text or dial 2-1-1 to access the support you need.

About the South Georgian Bay Ontario Health Team 

The SGB OHT is a new model of organizing and delivering health care that better connects patients and providers in their communities to improve patient outcomes.
