Dear EAP/SFBA community,

As we mentioned in our September update, we are in the midst of an organizational assessment. This is the part where we turn to you for your input. It would be tremendously helpful to us if you would share your feedback and thoughts with us by filling out this short survey. The survey should take about 10 minutes. Your responses are anonymous. Our consultants will be using this information to support our learning and growth.

Please dive in and let us know what you think. Honest feedback is greatly appreciated!

Submit your survey.
Deadline to submit:  Friday, November 6th, at 8pm.

Please get your input in soon!

Thank you,
EAP/SFBA Staff
