Patient Request for Health Information Model Form
As part of PHIMA’s involvement in the AHIMA HIM Awareness Campaign, we want to make our members aware of the Patient Request for Health Information Model Form.
The model form is intended to help healthcare providers streamline the patient health information request process and provide needed clarify on their obligations with respect to individuals' right of access under the Health Insurance Portability and Accountability Act (HIPAA). It may be modified with the healthcare organization's specific contact information.
AHIMA developed the plain-language form after hearing from healthcare and patient advocacy groups that consumers are often confused by inconsistent patient access forms, and from AHIMA members who have also asked for additional guidance on patients' right of access under HIPAA. The form and AHIMA recommendations align with recent recommendations by the Office of the National Coordinator for Health Information Technology (ONC) for a more streamlined and transparent records request process for consumers.
